A Case of Type 2 MI with Normal Coronary Arteries: Case Report & Literature Review

Clinicians have long recognized that acute myocardial infarction (MI) can occur in the absence of atherothrombosis . The Universal Definition MI Global Taskforce introduced a classification system 2007 (and reaffirmed 2012) defined type 2 (following standard diagnostic criteria) as occurring due to an imbalance oxygen supply and/or demand not caused by atherosclerotic plaque disruption. Nevertheless, ambiguity remains regarding how diagnose and distinguish it from both 1 injury. Here we report case 23 year old young woman attended emergency department, with typical chest pain shortness breath for 6 hours, Diarrhoea days, single time loss consciousness 5 minutes, hours before attending hospital. Cardiac enzymes were rising titres subsequent samples, Serum Creatinine was also high. Echocardiography performed 36 hour later, showed no regional wall motion abnormality, coronary angiogram normal arteries. So, diagnosis Myocardial Infarction (Type MI) Non Obstructive Coronary Artery (MINOCA) made, MINOCA attributed hypovolemic shock (resulting Dirrrahoea), manifested MI, Syncope AKI.
